MEMBERSHIP INFORMATION
A. Required materials for all AFFILIATE and PROVISIONAL membership applications:
- completed Membership Application Form
- an annual membership fee- $275
B. Required materials for all HOUSE membership application:
If the program is not self-governing but rather an extension of
a non-profit hospital or other sponsoring program as noted on the
membership application, verification of that affiliation must be
submitted.
- completed Membership Application Form
- completed Membership Update Form
- proof of Internal Revenue Service Code 501c3 non-profit status.
- house Mission Statement
- an annual membership fee- $425.00
C. Dues
- Membership dues are billed on an annual basis sixty days prior
to the anniversary date of your membership to NAHHH. Your anniversary
date is determined from the date we deposit your first membership
dues payment.
- Membership dues are due each year on your membership anniversary
date. NAHHH will send an invoice sixty days in advance of your
anniversary date.
- A Provisional Member may upgrade to House Member when your house
opens by submitting the required documentation and paying the
additional $50, or you may wait until the anniversary of joining
as a Provisional Member and renew as a House Member at that time.
